Appointments

We require a 48-hour notice to cancel or re-schedule an appointments. We understand that circumstances change and unexpected family commitments can alter every day plans. Please let us know as soon as possible so that we may offer your appointment time to another family that may be waiting.

If an appointment is missed, and we do not hear from the responsible party / parent the child’s appointment will be logged as a “failed appointment”. Two failed appointments will be forgiven but three may result in the family being dismissed from the practice.

Child Style Dental, LLC reserves the right to charge a $25.00 fee for broken appointments.

Financial

The staff is happy to assist with any insurance questions and will file all dental claims.

At the time of your first appointment we ask that you bring all insurance information and copy of ID.

Insurance benefits given over the phone are only an estimate and payment is never guaranteed. Any remaining balance not covered by the insurance carrier will be the responsibility of the parent / responsible party.

Treatment plans and financial responsibilities will be discussed with the responsible party / parent prior to beginning treatment. Treatment options for comfort will also be discussed at this time.  Dr. Arquitt is happy to offer Nitrous Oxide "laughing gas" for comfort or outpatient sedation if needed.

Care Credit

Care credit is offered to any family wanting assistance or payment arrangements. The application can be completed in the office in just minutes and an answer is only a phone call away.

Payments are interest free for 12 months.
